Krastinov's Bag of Horrors drops from Doctor Theolen Krastinov, a rare spawn in Scholomance on Heroic difficulty. He has a chance of appearing after defeating Rattlegore with a yell: The doctor is in! The toy is required for the 
[Memory of Scholomance] achievement, which unlocks the Legacy of Scholomance, the pre-Mists of Pandaria version of the dungeon.
Heroic has a daily lockout, so it can be farmed once per day per character.
