A Voidwalker was nearly summoned by a warlock (either Hubert Lucas or Sprag Buzzboom) near the Darkmaul Bridge on Exile's Reach. However, the warlock failed his binding ritual and was killed. When members of the Alliance or Horde came through the area on a rescue mission, they sent a warlock adventurer from their group to investigate the portal that the first warlock left behind.
The adventurer found the first warlock's grimoire and successfully completed the ritual, pulling the Summoned Voidwalker from the portal and binding it to their will.
