Partially Destroyed Diary is found deep inside the Obisidan Throne of the Obsidian Citadel in the Waking Shores. After entering at [27.2, 60.9], take a left at the Shas'ith fork, then enter the first room on the left. The Odd Book is at [23.54, 60.01] on a bookshelf in the right of the room as players enter.
The item begins the quest 
[70] Memories Revived, which has a prerequisite of completing [60-62] Stay a While and [60-62] Memories from Veritistrasz at the Ruby Lifeshrine.
