Though the Old God Yogg-Saron was slain at Ulduar, his unearthly body continues to threaten Azeroth. In its death-throes, the flesh of Yogg-Saron has given birth to monsters all throughout Northrend's underworld. His black blood flows through the caverns of the underworld, becoming the dark metal saronite, spreading the polluting insanity of the Old God far and wide.
The aqir, in their moment of triumph, have drawn out the vast carcass of the Old God and crafted it into their ultimate fortress - a bastion of black metal and divine flesh. Within, the aqir and all their allies - the Twilight Cult, the Arach'layn, and the legions of the Faceless, all await the crowning of the King of Darkness, the ultimate champion of the Old Gods. When the king rises, he will open the way for the Unspoken, the Old God of Silence, to return to Azeroth.
Then, she will open her six mouths, and sing the song that ends the world.
Dungeon Journal
Rising up from the darkness of the underworld is the Undying God, the eldritch carcass of Yogg-Saron. Though emptied of the Old God's eternal essence, the vile and sprawling corpse of Yogg-Saron still writhes in unearthly death throes, its flesh turning to metal and twisting into a dreadful new shape. The body of Yogg-Saron has become a fortress, a bastion of insanity that threatens to crack apart the crown of the world and spill the legions of the aqir back into the world.
From within the heart of the Undying God, the aqir and the King of Darkness carry out the rituals to free the Unspoken from her confinement. If they succeed, it will mean the rise of an unstoppable Old God, the triumphant return of the aqir, and the end of order and sanity for all of Azeroth.

Yogg-Saron - The King of Darkness
Through the black rituals of the aqir, the undying essence of Yogg-Saron has been channeled into the vacant carcass of Arthas Menethil. Tempered by the immense powers of the Lich King, this shell is the only vessel suitable to house the spirit of an Old God. Now, the fiend of a thousand faces wears a single mask. He is the King of darkness. For how could the ultimate champion of the Old Gods be any other than one of their own? He will shatter the barriers keeping the full terror of the Old Gods from this world. He will usher in the final silence.
The Unspoken
The King of Darkness has opened the way for the Unspoken. Silence will fall, and she will sing the song that ends the world.
Aftermath
With the defeat of Yogg-Saron, the King of Darkness, the Unspoken is forced back into her prison dimension along with her aqir followers. Yogg-Saron is cast into the void along with the Unspoken, still inhabiting the body of Arthas Menethil, howling in defiant fury. The Undying God crumbles around the defenders of Azeroth, unable to maintain its structure without the King of Darkness present. The aqir spirits that inhabit the fortress are pulled screaming into oblivion as their scheme unravels all around them.
Ultimately, the watchers of Ulduar led by Tyr arrive to aid in the escape of the heroes. Transporting everyone safely to Ulduar, Tyr recounts the defeat of the aqir and their plan to unleash the Unspoken upon Azeroth. The darkness beneath the world has been dealt a grievous blow, with two of the Old Gods forever banished from the world. However, the nerubian Newbreed still persist, burrowed deep into Azeroth's underworld, and they will likely remain an ever-present threat to the safety of the surface.
Nevertheless, the heroes of the world who champion order and peace have won a great victory. There will be time enough to turn attentions towards other threats later...
