I'm not entirely sure about this. On patch day I fought this guy on Heroic and my group beat him without a total wipe. A couple of days later, on another character I got random heroic for pits of saron with only this guy left. It is my belief that what you leave alive in the first part of the instance will come up to fight the npcs. The group had left a lot of castered alive that were aggroing our group and picking us off.
