[Stygia] after retrieving 
[Crumbling Stele]. It is part of the 
[Bound Shadehound] acquisition.
After using 
[Rune Codex Page: Forging], [Rune Codex Page: Souls], and [Rune Codex Page: Binding], the item transforms into 
[Intact Rune Codex].
