Lingering Plague is a conduit for unholy death knights. It drops from Domina Venomblade in Plaguefall and Sylvanas Windrunner in the Sanctum of Domination.
It can also drop from various Shadowlands rare spawns, world quests, and Covenant Callings, scaling to your item level.
