The Key to the Focusing Iris drops from Sapphiron in the 10-man version of Naxxramas. Although it is a quest item, it must be rolled for, meaning only one person in the raid will be able to loot it, receive the quest 
[30] The Key to the Focusing Iris, and receive an item Level 213 quest reward item for completing the quest [30R] Judgment at the Eye of Eternity, which involves getting a raid to kill Malygos.
