The Heroic Key to the Focusing Iris drops from Sapphiron in the 25-man version of Naxxramas. Although it is a quest item, it must be rolled for, meaning only one person in the raid will be able to loot it, receive the Heroic Key to the Focusing Iris quest, and receive a item Level 226 quest reward item for completing the Judgement at the Eye of Eternity Quest, which involves getting a raid to kill Malygos.
