Patch changes

Patch 5.0.4 (2012-08-28): Removed.
Patch 4.0.1 (2010-10-12): Reduced to 2 ranks, with a 50/100% chance (was 3 ranks with a 33/66/100% chance). Moved from tier 4 to tier 2.
Patch 3.0.2 (2008-10-14): Moved to Tier 4, increased to 33/66/100% chance (was tier 3, 25/50/75% chance).
Patch 1.12.0 (2006-08-22): Now has 3 ranks (was 5 ranks). Chance to add a combo point is now 25/50/75% (was 15/30/45/60/75%).
