It is on the leftmost pile of bones when entering the lower level of the Great Ossuary through the door, where Rattlegore is encountered. Turn it into an  [Argent Dawn Commission] by repairing it with a  [Righteous Orb] and four  [Truesilver Bar]. It will only appear if the completed trinket or a variation isn't present.
