Clicking the gossip option makes a warning popup appear:

Accepting the prompt will immediately save the player to the instance and they will teleport to the Inner Sanctum, where Garrosh awaits.
Patch changes
Patch 10.1.5 (2023-07-11): Players that have defeated Garrosh Hellscream in Siege of Orgrimmar may now skip directly to The Inner Sanctum on all characters on the account by reading the scroll hanging on the wall near Loremaster Cho[sic] at the entrance.
