The Inner Demon spawns after meditating at the Shrine of Thunder at the Eternal Spring in Vol'dun during 
[30-60] Cleanse the Mind. Visually, it is a red-tinted, translucent copy of the player character's model.
Abilities
Inner Strength — Unlocks the strength within, increasing damage done by 50% for 7 sec.
Inner Torment — Torments the target, inflicting Shadow damage.
