This item can occasionally be found on a pile of treasure in Neltharion's Vault. Your Artifact Knowledge must be at least Level 5 to see this item. If the Burning Plate of the Worldbreaker is present, you will see the emote "You hear a strange roar from the cavern ahead" when you enter Neltharion's Vault. If you do not see this emote, you will need to try again the next day.
Note that you must be in Protection spec to see both the emote and the item.
You can use this macro the check if the item is available today:

If it returns true, switch to protection spec and visit Neltharion's Vault, and the item should be there.
