Blazefury Medallion dropped from Lord Kazzak in the Blasted Lands prior to The Burning Crusade and later during the Dark Portal Opens world event from Highlord Kruul (who had the same loot table).
A timewarped version drops from Lord Kazzak (Anniversary) during WoW's yearly anniversary event.
