The Forgotten Key drops from end-of-wing bosses in Torghast, Tower of the Damned. Characters who leveled through the normal storyline will receive it from Warden Arkoban during 
[60] The Search for Baine.
Characters who leveled through [60] The Threads of Fate will receive the key from the first boss they defeat after reaching level 60. However, it can still only be used while on, or after completing, [60] The Search for Baine.
As a quest objective
This item starts [60] Prison of the Forgotten.
