This item can be found in two different zones:
Elwynn Forest
Look for Polymorphed Apprentices throughout the zone. Cast 
[Polymorph] on them to turn them back into Servants of Azora, who will leave 
[Azora Apprentice Notes] on the ground. Once six of these are obtained, use them to create the Spell Notes.
Tirisfal Glades
Look for Odd Melons near the Solliden Farmstead. Cast [Polymorph] on them to turn them back into Forsaken, who will leave [Apothecary Notes] on the ground. Once six of these are obtained, use them to create the Spell Notes.
