The Living Cowl drops from Molt Thorn in the Swamp of Sorrows.
This item was dropped by the Tangled Horrors that were located in Swamp of Sorrows prior to Cataclysm. It became available again during Warlords of Draenor through 
[Crate of Salvage] and 
[Big Crate of Salvage], but the salvage crates were later removed upon the conclusion of that expansion.

Patch 8.0.1 (2018-07-17): Now drops from Molt Thorn. 
Patch 7.0.3 (2016-07-19): No longer drops from salvage crates.
