The Head of the Obliterator can be found east of the Cauldron in the Searing Gorge.[65, 45]
Assembling it together with the 
[Left Arm of the Obliterator], [Right Arm of the Obliterator], 
[Left Foot of the Obliterator], and [Right Foot of the Obliterator] will summon Iodax the Obliterator.
Notes
Though the head is not looted, there is an unused item ID for it in the database.
