Efrat's Forgotten Bulwark is found at [78.0, 48.9] under the bridge in North Sufaad while Phase Diving in K'aresh. Players are required to have the "What Lies Beyond" perk purchased for the 
[Reshii Wraps] to see it. This is one of the criteria of 
[Treasures of K'aresh]. If the appearance of this item is already known, 
[Efrat's Totally Abandonded Bulwark] will appear in its place.
